Fees & cancellation policy

NDIS | DVA I MY AGED CARE | PRIVATE PAYING CLIENTS 

Coree Occupational Therapy provides client-centred services on a fee for service basis. As a non-registered NDIS provider we support NDIS participants who are plan or self-managed. We are not bound by the NDIS price guide.  
OCCUPATIONAL THERAPY

  • Initial appointment fee of $426.00 exclusive of travel, report writing and indirect services.

  • Capacity building supports – occupational therapy review assessment and intervention rate $213.00 p/h

  • Improved daily living skills – Assessment and intervention $213.00 p/h

ALLIED HEALTH ASSISTANT

  • Improved daily living skills – Allied Health Assistant $95.46 p/h

  • Capacity building and supports – personal training $95.46 p/h

TRAVEL

The Modified Monash Model (MMM) is a geographical classification system. It identifies travel boundaries and maximum travel charges.

REPORT WRITING 

  • Charged at – $213.00 p/h

  •  Initial / Progress Report / NDIS Functional Review – minimum 10 hours

  •  SIL or SDA – minimum 25 hours

  •  Home Modification Report – minimum 20 hours

  •  Assistive Technology Report – minimum 8-12 hours

  •  Functional Assessment – minimum 10-15 hours

OTHER CHARGES

  • Initial file set up and triage of referral – 1 hour - $213.00 p/h charges

  • Nordic Walking three session beginner class + poles

  • Ultimate Walking Group $250 6-week block.

CANCELLATION GUIDE

Early cancellation: The client or a guardian cancel the appointment by contacting Coree Occupational Therapy 48 hours prior to their appointment on 0499 923 152

Late Cancellation: The client or a guardian cancel the session by calling Coree Occupational Therapy with less than 48 hours until the appointment however, charges apply.

Missed appointment/No show: When a client does not turn up to their booked session at the clinic, in person, via tele-health or don’t notify Coree Occupational therapy that they are not at attending at the time of an appointment for whatever reason.

CANCELLATION FEES

  • Early cancellation: No fee
    Late cancellation: 100% session fee

  • Missed appointment/No show: 100% session fee

  • Coree Occupational Therapy reserves the right to void the privilege of holding a specific time for clients or families who repeatedly do not attend or cancel their appointment, regardless of the amount of notice provided.

CANCELLATION PAYMENTS

An invoice will be created and emailed following a late cancellation or no show, which is to be paid prior to or at your next scheduled appointment

Use of NDIS for missed/cancelled appointments: Please refer to our service agreement

For all clients that have appointments paid by an outside organisation, Coree Occupational Therapy has the responsibility to all of these organisations to use the funds for therapy and be accountable to outcomes. We would prefer to work with you to achieve your goals.
